Complaint Form

The Commission takes all complaints seriously. When we receive a complaint, it will be shared with the executive director to decide the appropriate action to take on a case-by-case basis. The Commission staff will respond to the complainant within five business days.

Please only use this form to complain about a board member, staff member, or contractor. For any other matter, please contact a staff member directly.
