The purpose of this survey is to collect input from Cherry Creek students, staff, and parents/guardians within the Overland and Horizon feeder areas. Two STRIDE school-based community health centers opened in early 2022 (at Overland HS/Prairie MS and Horizon Community MS campuses) and in addition to providing healthcare services, these clinics will also offer health education opportunities that will complement the health education curriculum already being taught within the schools. Your feedback is essential in our ability to effectively meet the needs of the community and to develop relevant health education programming. Please note that participation in this anonymous survey is completely voluntary and lack of involvement will not preclude you from seeking care at one of these school-based community health centers.
