You have reached the DORS Accommodation Portal for submission of Reasonable Accommodation Request forms for employees of the Division of Rehabilitation Services, Maryland State Department of Education. This portal is secure and all information provided will be used only for determining whether you qualify as an individual with a disability under the Americans with Disabilities Act and for use in the Reasonable Accommodation interactive process. All information will be kept separate from your personnel records.

If you have it available, please include medical documentation for the impairments for which you are requesting accommodations. Sufficient medical documentation should describe the nature, severity and duration of the impairment, the activity or activities that the impairment limits, and an indication of why the reasonable accommodation is needed to perform the essential functions of your position. This is especially needed when the disability and/or need for accommodation is not known or obvious. Please only include medical documentation relevant to the impairment(s) for which you are requesting an accommodation.
