MHA Keystone Center - Addressing the Unique Care Needs of Older Adults

As a patient safety and quality organization committed to better outcomes, the MHA Keystone Center is dedicated to supporting health systems across Michigan in becoming age-friendly. By adopting evidence-based models and delivery care practices that meet the needs and preferences of older adults, we not only improve care across the care continuum but also across the continuum of life. The center has launched two state-based Age-Friendly Health Systems Action Communities, which enable healthcare teams to accelerate the reliable practice of the evidence-based 4Ms Framework in an active community of learners and testers. This survey assesses health systems’ interests and needs for ongoing support around improving care for older adults. The results of this survey will determine priorities and will help shape and guide the MHA Keystone Center’s future older adult care offerings.
