This form is for use by DGSOM student leaders and/or society ambassadors to register an event with the SAO and request food and/or supplies for their event. This form is mandatory to request SAO to place orders for SAO, MSC and Society funded events.

You must submit this form a minimum of 2 weeks before your event.
All events must be approved and posted in the Medical Student Calendar.

Student Organizations:
Please note SAO funding can be used August 4th-Dec 31st of the 2021-22 academic year, and groups holding talks after Dec 31st should apply to the MSC for event funding.

This list will be used to order food for your event and submitted to Accounting. $7.50 per person x up to 50 people can be funded for a total of $375.
