This short form is for the following people/organisations to complete:
- Nurse interested in future TPP programs
- Workplaces looking to employ a nurse in primary health care
- A PHN wanting to fund positions in the program
This form will let us know that you are interested in participating, employing or funding nurses in any upcoming Transition to Practice Programs.
Important: This is not an application for the program. The information you provide us will aid us in making informed decisions on future programs.
- Nurses interested in future programs: We will contact you if you may be eligible to apply for future programs
- Workplaces looking to employ a nurse in primary health care: We will contact you when an opportunity to apply for funded job advertisements becomes available
- PHNs interested in funding positions: We will be in touch once you have set a meeting with our team
