The 2022 Childhood Obesity Prevention/Environmental Health and Sustainability Awards

Application Instructions


Please read the application guidelines below before beginning this application.

Information provided in this application will be used to select recipients of the 2022 Childhood Obesity Prevention/Environmental Health and Sustainability Awards. In addition, the information provided in this application may be highlighted on The United States Conference of Mayors (USCM) website and social media channels, in best practices publications, and/or shared via other media for educational or other purposes.


Awards will be given in three population categories – small, medium, and large city.

  • Small City – Population smaller than 75,000  
  • Medium City – Population 75,000 – 250,000
  • Large City – Population greater than 250,000

In order to be eligible, a city must meet the following baseline criteria:
  •  The city must be a member of The United States Conference of Mayors.
  •  The application must include a letter of support from the Mayor.

The application deadline is 11:59PM EDT on Friday, October 22, 2021.
 

The application itself has eighteen (18) questions in total.  Four of them - Questions 11, 12, 13 and 14 - are essay questions; questions 15 and 16 require file uploads; most other questions ask for contact and other basic information.  Only Question 17 is optional; all other questions MUST be answered in order to consider your application complete. You will not be able to submit your application unless all required questions have been answered.
