Spring Commencement 2022 Request Form

This form is NOT REQUIRED if your expected graduation quarter is listed in your portal as winter 2022, spring 2022 or summer 2022. The deadline to complete this form is April 3, 2022.

Please only complete this form if:

• You would like to participate in a commencement ceremony that is different than the one you are automatically scheduled to participate in (learn how to check your expected graduation term).
• You are in a Blended Program and would like to participate in a commencement ceremony at the completion of the undergraduate portion of your program.
• You are an undergraduate student and recently changed your graduation term.
• You are a graduate student and submitted your Application for Graduation form late (less than one quarter prior to the quarter in which you will graduate).

Please note that requesting to change your commencement ceremony is not the same as changing your graduation term.

Commencement programs are created for each ceremony and list the names of eligible graduates. If you meet one of the conditions above, please complete the following form by April 3. to ensure that your name is included in your ceremony program and to receive ongoing commencement communications.
