This is a survey for families with children in
grades K-12 receiving special education services. This is your opportunity to provide input to improve services and outcomes for children and families in North Dakota. Your responses are extremely important to us.
Thanks for taking the time to complete the survey!You may have already completed the survey (for example after your child’s IEP meeting or at another event). If you have already completed the survey for the 2023-24 school year, please do not complete the survey again. If you have more than one child receiving special education services in grades K-12: The school should have indicated for which child you are to complete this survey. If you are unsure as to which child, please follow-up with the school.
