This survey is intended for sponsors of the Child and Adult Care Food Program (CACFP) operating in Washington state. It has 2-pages and will take approximately 5 minutes to complete.
Please complete the survey whether you intend to participate in Farm to CACFP Week (October 4-8, 2021) or not. For those that do intend to participate, we will ask if any areas of technical assistance are needed to support your efforts. For those that do not intend to participate, we would appreciate your feedback as to why.
Sponsors that participate in Farm to CACFP Week can expect to receive another survey in mid- to late October to collect information about local foods served, enrichment activities offered, and other information that will help us improve our support in future years.
