The purpose of this Evaluation Tool is to aid you as a leader in evaluating your hospital or clinic in this particular focus area.
It is meant to help you to begin some strategic planning and to determine:
Where are we now?
How are we doing?
What are our internal strengths and weaknesses in this area?
What are our external opportunities and threats?
What has changed in this area during the last year, both within our organization, and how are we responding to things that may have affected us externally?
Are we reaching our goals, or do we need to adjust/ reset goals?
By submitting your information, it will help you to identify areas that may need attention within your focus area. It will also be confidentially shared with the trainer or subject matter expert in order to tailor the follow up training held in December 2021. Finally, it may help you to identify that this is an area you'd like some outside help or support through this grant program.
Please submit your responses no later than October 31, 2021 so that they can be reviewed and incorporated into upcoming training.
