Voucher Outcome Survey

You were issued a voucher by the Philadelphia Housing Authority (PHA). We have not yet received a request for lease approval and your voucher will soon expire. So that we can better understand the challenges faced by families when trying to locate and lease units in the Voucher Program, PHA is requesting that you complete this short survey. We will use this information to assist us in developing programs and practices to help applicants successfully locate and lease units. If you complete this survey within 10 business days from the date on your Voucher Expiration Notice, PHA will provide you with an automatic 30 day voucher extension.

The survey must be completed in one session. You will not be able to re-visit the survey once you click submit.
