This is a survey of health care experiences and needs by the  Mendocino Coast Health Care District, a public agency. Please participate if you or someone you know used the Coast Hospital or the clinic located on the hospital campus. This is not a survey of other practices or clinics in the area such as the Mendocino Coast Clinic.
