GMU COVID-19 Survey

Introduction & Consent

This research is being conducted as part of a COVID-19 vaccine education initiative and doctoral research project. We are interested in learning about your opinions, experiences and preferences related to the COVID-19 pandemic and COVID-19 vaccine.
If you agree to participate, you will be asked to complete a 15-minute survey. Your responses will help us make sure we can develop COVID-19 vaccine information that will meet the needs of people like you. 

There are no foreseeable risks for participating in this research.

There are no benefits to you as a participant other than to further research in COVID-19 vaccine communication.

The data in this study will be confidential. De-identified data (like aggregate datasets) could be used for future research without additional consent from participants. Also, our reports will not include your name.

The Institutional Review Board (IRB) committee that monitors research on human subjects may inspect study records during internal auditing procedures and are required to keep all information confidential.

To participate in this study you must be an adult (18+ years) and a current student, faculty or staff member at George Mason University.

Your participation is voluntary, and you may withdraw from the study at any time and for any reason. If you decide not to participate or if you withdraw from the study, there is no penalty or loss of benefits to which you are otherwise entitled. There are no costs to you or any other party.
