REFERRAL INFORMATION
This form is to be completed by the referring agency and returned to Partners for Youth. Information on this form will be kept confidential (except in such cases as required by law) and will be used to assist the Case Manager in matching the child with an appropriate Mentor. As a general rule, the more we know about the child, the more quickly and successfully he/she/they will be matched. We will contact the family to obtain additional information.
