Community Health Survey

Community Health Improvement Survey for Ashland, Bayfield, and Iron Counties

Memorial Medical Center (MMC) and the Ashland, Bayfield, and Iron County Health Departments are working together to conduct each organization’s Community Health Needs Assessment (CHNA). 
 

One of the first steps in conducting the 2021-2023 CHNA is to gather feedback from the community and gain support by asking residents to complete a short survey.


This survey is specifically to learn about and improve health issues in our region. Participation is voluntary. Your answers will be anonymous and combined with those of all other survey respondents. The results will be shared with community members who are interested in improving the health of our community. The survey should take about 5-7 minutes.
1. Which zip code do you live in?
The following are health focus areas that may impact the health of community members. Below are descriptions to explain each of the 12 focus areas.
  • Substance Use (alcoholism, drug use and abuse, opioids, impaired driving, alcohol/drug related injuries or death, access to alcohol and drug treatment programs)
  • Chronic Disease (diabetes, cancer, heart disease, lung disease, obesity, etc.)
  • Communicable Disease (tuberculosis, tick-borne illnesses, COVID-19, whooping cough, food-borne illnesses, STI's, chickenpox, measles)
  • Environmental and Occupational (safe supply of food and drinking water, disposal of toxic wastes, air/water/noise pollution, safe work environments)
  • Growth and Development (optimal opportunities for physical, behavioral, cognitive and emotional growth and change across the lifespan)
  • Injury and Violence (recreational activity injuries, car accidents, battery, assault, gang-related violence, domestic and elder abuse, homicide)
  • Mental Health (suicide, depression, other mental disorders; access to mental health professionals,)
  • Nutrition (access to healthy foods at reasonable prices, education regarding nutrition)
  • Oral Health (access to dental services)
  • Physical Health (access to workout facilities, public recreation, walking/biking trails, weather conditions)
  • Reproductive and Sexual Issues (access to family planning services, birth control availability, STD education)
  • Tobacco Use (access to tobacco, vaping, education about health implications, access to cessation programs)
