COVPN3006 Screener Version2 June 2021

Thank you for responding to our ad for COVID-19 vaccine research.

Project ACHIEVE is looking for volunteers aged 18-29 years to participate in the PreventCovidU research study to learn:
-whether the Moderna COVID-19 vaccine protects people from getting infected with SARS-CoV-2 virus (which causes COVID-19 disease)
-if getting the vaccine prevents transmitting SARS-CoV-2 infection to others.

We already know that this vaccine can prevent serious COVID-19 disease, but we do not know if the vaccine will prevent the virus from shedding from the nose and mouth and infecting others.

The COVID-19 Prevention Network (CoVPN) is conducting the study in cities across the country. The study will enroll 18,000 people between the ages of 18 and 29 years, who have not previously been infected with SARS-CoV-2, and who have not yet received a COVID-19 vaccine. You do not have to be a college student to enroll into the study.

If you decide to join the study, you will be assigned into one of three groups.

The first two groups will be randomly selected to one of the following: (1) half will receive the vaccine injections right away, or (2) half will receive the vaccine 4 months later.
The third group will include people who prefer not to get the vaccine and will be followed for 4 months.

During this 5-month study, you will receive COVID-19 vaccines (if you are in the first two groups), answer daily questionnaires in an app, take daily swabs of your nose, provide blood samples at 3 timepoints, and follow additional procedures if you test positive for SARS-CoV-2. You will be asked to identify close contacts to be involved in the study to help us learn if the vaccine given to you prevents transmission of the virus to others.

If you prefer not to get the vaccine, you can still enroll into this study.

You will receive up to $950 total if you complete all visit procedures.
