CERTIFICATE OF INSURANCE
A Certificate of Insurance (COI) from the appointed contractor must accompany this form. The COI should be issued in their co.
name, and meet the following requirements:

1. Policy Holder: Informa Markets Manufacturing LLC, located at 2901 – 28th Street, Santa Monica, CA 90405
2. Workers’ compensation must provide at least $1,000,000 in coverage for each accident.
3. Comprehensive general liability must provide at least $1,000,000 in coverage for each accident.
4. Certificate must name Informa Markets Manufacturing LLC, The Anaheim Convention Center, City of Anaheim, Anaheim
School District, Community Center Authority, and Freeman as additionally insured.

Failure to meet these requirements will result in denial of access to the show floor.
