Diabetes Survey


Welcome, and thank you for participating in our survey! Currently, more than 21 million people in the United States have type 2 diabetes, and more than 50% of people with type 2 diabetes are not meeting their target blood glucose (sugar) levels. The goal of this survey is to help us understand how comfortable you are working with your healthcare provider to set and meet goals. Your responses are anonymous and will be used to develop educational activities that help doctors improve the way they talk with their patients about type 2 diabetes.
