Badge Request Form

IMPORTANT: Read the Badge Creation Requests webpage before submitting.

Please allow up to 4 weeks for completion. All requests are subject to a review process and approval by staff.

If you are requesting multiple badges, please fill out a separate form for each badge so we can clearly understand the different requirements. If you have any questions about the request form, please contact info@mountaineers.org.

Beginning in Fiscal Year 2025, Student Course badges will only be created for courses where a student badge serves as prerequisites for other activities. This update is intended to streamline available badges and emphasize essential skills for progression in courses and activities. *This question is required.

5. Timeline

When are you hoping to have your badge completed? Note that it generally takes 2-4 weeks to get a badge created on the website. Please submit your request at least 4 weeks in advance of the date you need it completed.

6. Who will be assigning the badge from each applicable branch activity committee? *This question is required. The individuals listed here will receive administrative privileges for the badge page, allowing them to assign badges. Each applicable branch activity committee must designate a badge administrator to be included for approval.

7. Badge Template

Click to access the Badge Template Folder in Google Drive and follow these instructions to complete your badge template.

Select the appropriate badge template for your badge type. This will open a Google Document.
Make a copy of this document and save in your Google Drive.
Rename the document with the title of the badge.
Complete all sections of the template.
Paste the sharing link to your document below:
1. Click 'Share' on the Google Document.
2. Select 'Get Link'.
3. Set the sharing so that anyone with the link can edit.
4. Copy the sharing link and past the link in the text box below.

All completed badge templates should be returned in Google Doc format. This will make it easier for us to work with you if we need to make edits and revisions.
