Please go back to Question 4 and select Initial Loan Request. A 2021-2022 Loan Request Form must be submitted and processed by Student Financial Services before a 2021-2022 Loan Change Form can be accepted.
Please go back to Question 4 and select Loan Change Request. A 2021-2022 Loan Change Form is required to make any loan adjustments after an initial loan request has been disbursed.
By completing this application you are confirming you are requesting Relay to evaluate, process, or update a Federal Direct Unsubsidized Loan on your behalf for the 2021-2022 academic year. 

Federal Direct Unsubsidized Loans are federal funds you can borrow to help pay for the costs of your Relay education. These loans do accrue interest while in school. The loan principal, origination fee, and interest will become payable 180 days following exit from Relay. Graduate students can request up to $20,280.00 (net) annually, in combination with other federal aid such as TEACH Grants. A net loan is a student's loan request before a percentage of the loan, set by the federal government, is added onto the requested amount.

Disbursement dates for each term for the 2021-2022 academic year
  • Summer: July 15, 2021 and August 5, 2021
  • Fall: September 23, 2021 and November 18, 2021
  • Spring: February 17, 2022 and April 21, 2022
Deadlines to request federal financial aid or request changes to applications for the 2021-2022 academic year
  • Summer: August 15, 2021
  • Fall: December 6, 2021
  • Spring: May 22, 2022
All federal financial aid documents must be received no later than the dates listed in order to process your federal aid request on time.

I confirm that I am not requesting federal financial aid from another institution. Or, if I am requesting federal financial aid from an institution, I confirm that I have already notified Relay Student Financial Services.
I understand that my Federal Direct Unsubsidized Loans include an origination fee from the US Department of Education, and that the fee will be automatically added to the amounts I request. The 2021-2022 origination fee for Federal Direct Unsubsidized Loans requested before October 1, 2021 is 1.057%. The new origination fee starting October 1, 2021 for the 2021-2022 academic year will remain at 1.057%.
I understand that my loan request for each term will be divided into two equal installments. I understand that any credit refunds due to me will take up to 14-days to be sent by mailed check or direct deposit.
I understand that if I withdraw or reduce my enrollment to below half-time status before the end of the academic term, I will be required to return any unearned federal aid funds to Relay. I know that I must attend my classes or submit coursework within 30 days of my disbursement date in order to be eligible for a disbursement.
I authorize Relay Student Financial Services to link my academic year 2021-2022 requested Federal Direct Unsubsidized Loans to a prior active Master Promissory Note, if applicable. Relay will notify me if my prior Master Promissory Note has expired or is close to expiration.
I understand that if I am due a credit refund and I do not select a refund method by the deadline, Relay will deliver the refund via a physical check. I authorize Relay to mail the check to the address on file with the Office of Academics and Enrollment. I know that I can make another refund method selection at any time on the Nelnet refund portal. I know that I can find the terms and conditions of the account on the Nelnet website. And I understand that Relay may provide information about me to the refund provider in compliance with federal regulations.
I understand that my loan request for each term is automatically applied to that given's term balance only.

I confirm that I am not requesting federal financial aid from another institution while enrolled at Relay. Or, if I am requesting federal financial aid from another institution I confirm that I have already notified Relay Student Financial Services.
I understand that my $2000 TEACH Grant is subject to the US Department of Education's sequestration fee and that the total award will be disbursed equally across terms, two equal disbursements per term. For the 2020-2021 academic year, the sequester fees for TEACH Grants requested before October 1, 2020 is 5.7%. The new sequester fee starting October 1, 2021 for the 2021-2022 academic year will remain at 5.7%.
I understand that I must sign an Agreement to Serve, in which I agree to teach a high-needs field in a low-income school for 4 of the next 8 years full-time after graduation.
I understand that if I switch into an ineligible major or program, I will not receive the undisbursed portion of my TEACH Grant and I will remain responsible for completing my service obligations for any TEACH Grant funds already disbursed.
I understand that if I withdraw from Relay, my TEACH Grant will be prorated per federal regulations.
I understand that I can request that my TEACH Grant be converted into a Direct Unsubsidized Loan at any point and it cannot be converted back into a grant under any circumstances.
I understand that if I do not comply with the Agreement to Serve, my TEACH Grant will be permanently converted into a Direct Unsubsidized Loan.
I understand that I can request that my TEACH Grant be converted into a Direct Unsubsidized Loan for any reason and I am responsible for paying back the full amount as well as all interest accrued from each disbursement.
