SDoH Screening - Automated Engagement

Overview

Alliance for Better Health (Alliance) is seeking an automated community member engagement service to conduct Social Determinants of Health (SDoH) screenings. This solution is a direct response to the growing body of research demonstrating that addressing social determinants of health more effectively in underserved communities can improve health while lowering costs. We envision a solution that is straightforward and easy to use at the community member level.


Goal: 

Develop a cost effective and efficient way to connect with and survey community members on their social care needs.  

Requirements:
  1. A robust analytics and customizable reporting tool to provide insight into operations and outcomes.
  2. Process that connects with and receives responses from a significant percentage of members, while requiring minimal manual intervention.
  3. Customizable questions.
  4. Secure data exchange capabilities.
  5. Ability to scale.
  6. Proven engagement metrics based on similar programs.
Timeline:
  1. RFP window will open on April 21, 2021 and will close at 3pm ET on April 28, 2021.
  2. Review of proposals will conclude by May 21, 2021. 
  3. Demonstration of the product will be requested of qualified candidates.
  4. Notification to the successful candidate is targeted for mid-May.
  5. Target Product implementation July 2021.
  6. Alliance can alter the timeline at their discretion.
