Statewide Asthma Coalitions

The survey takes between 8 - 10 minutes to complete. After completing the survey, you will be able to enter your email in a separate form in order to be eligible for winning one of twenty $10 Starbucks gift cards that will be sent out. You will also be asked if you are willing to participate in an interview or focus group on the topic of sustainable asthma coalitions. You will only be contacted if you provide permission for us to do so.

We recommend setting aside 10 minutes of time in a quiet and uninterrupted space with a good internet connection to complete the survey in a single session since you will not be able to save a partially completed survey.

1. Our research is exploring both current and former statewide asthma coalitions. In order to ensure you have the proper survey questions provided to you, please answer the following:

Is your coalition "Currently Active"? A "currently active" coalition is defined here as: your coalition meets regularly (or did prior to COVID), meeting minutes are taken, work occurs within subcommittees, conferences or workshops are held, programs or initiatives are actively in progress, etc.

- OR -

Is your coalition "Formerly Active"? A "formerly active" coalition is defined here as: your coalition may have dissolved or merged with another coalition, it may meet informally or irregularly, minutes are not kept for meetings, officers are not elected, committees no longer meet, the CDC's National Asthma Control Program funding is no longer providing support, etc. *This question is required.
