Instructions:
All social impact measurement projects conducted by, or on behalf of Huber Social, or that use the Huber Social Wellbeing Measurement Framework, platform or tools must obtain approval from the Huber Social Ethical Review Board (ERB) before any measurement activities commence.
Prior to beginning the ERB project approval application, please review the Huber Social ERB project assessment guide found in the 'Tools' section of the Huber Social consultants portal.
As a reminder, applications for ethical approval must be made after the Measurement Plan (including the Social Impact Model) has been documented and agreed with the client, and at least a first draft of measurement tools has been developed. The Measurement Plan and tools (i.e. survey question set) must be submitted as attachments to the application.
All applications for ethical approval must be made using this Huber Social Project Approval application. Verbal applications or applications in other formats will not be accepted. Each question field must be completed and cannot simply refer to an attachment, although attachments may be used to support the information provided in the form.
The completed application, along with the Measurement Plan and any other attachments, will be submitted to the Huber Social Chief Integrity Officer (CIO), who is responsible for reviewing these materials and sharing with the ERB for external approval if the project is deemed 'high risk'.
An ethical review decision can be expected within 2-3 weeks of project approval application submission. Notification of project approval status will be sent electronically to the email provided in this application.
Please note that for ongoing measurement projects, all measurement plans and survey tools should be submitted for review at the same time so that the entire measurement program can be assessed at one time.
