The Central Michigan District Health Department is now making appointments for individuals who are 12 years of age and older for their COVID-19 vaccine appointments.
**This form is for FIRST DOSE appointments only, if you need a second dose appointment please call your local CMDHD branch office.**
Please schedule your appointment in one of our six counties that you wish to be vaccinated in. These counties are: Arenac, Clare, Gladwin, Isabella, Osceola and Roscommon. The vaccine provided at each location is based on our availability, and may change. We will try to offer two types of vaccine based on our vaccine supply.

- I agree to register for ONE appointment per person at ONE clinic location ONLY
- I agree to show up for my appointment, otherwise my vaccine will be forfeited
- I agree NOT to come to the COVID clinic if I am having any COVID like symptoms
- I agree that the appointment is being scheduled for someone who is 12 years of age or older.
