The Survey that follows will gather input for the Study. Please note the following:
• Respondents may "save and continue" at a later time by clicking on the Save and Continue button at the top of each page.
• All responses are confidential and neither identifying information nor specific responses will be disclosed or shared.**
• A Study Report will be provided to all respondent organizations.
• Respondents will be invited to participate in a webinar that summarizes the Study’s findings.
• Responses received by September 1st will be incorporated in the Study.
**Collection of identifying information is necessary to ensure the integrity of Survey results, for example the elimination of accidental duplicate responses. Identifying information and Survey responses are confidential and will not be disclosed or shared with third parties.
