Reporting Adult Influenza Vaccine Initiative (AIVI) Doses Administered
Welcome to the Texas Department of State Health Services (DSHS) Immunization Unit online survey to report the number of AIVI adult influenza vaccine doses administered at your facility.
- Every dose of AIVI influenza vaccine administered by your facility must be reported through this survey by the 5th of each month.
- You and your staff can select the best way to manage the immunization records in order to report the doses administered in your clinic.
