As an added safety measure for everyone, please complete this Kid's Club Wellness Check. This form is only intended for persons of at least 18 years of age.

PRIVACY NOTICE
In light of the unprecedented challenges associated with COVID-19, to reduce the risk of spreading the virus, and to protect our employees, contractors, and guests, Caruso is implementing screening procedures for participants in the Kid's Club program. Participants are required to complete a survey prior to visiting a Kid’s Club program.

As part of these screening procedures, we are collecting the following categories of Personal Information from Kid’s Club participants: name; email address; phone number; the number of guests in your party; your Kid’s Club Location; and medical and health information related to COVID-19 symptoms and exposure, specifically body temperature and whether anyone in your party is currently displaying certain symptoms such as cough, chills, repeated shaking with chills, muscle pain, new onset headache, sore throat, new onset GI symptoms, new loss of taste or smell or shortness of breath, whether anyone in your party has recently been in close contact with anyone who has recently tested positive for COVID-19, whether anyone in your party are awaiting results of a recent COVID-19 test, and whether anyone in your party has recently traveled outside the State or Country.

 We are collecting this information for purposes of reducing the risk of spreading the COVID-19 virus and protecting our employees, contractors, and guests.
