2021 HS Evaluation Cohort Application

Dear Healthy Start Grantees,

Healthy Start (HS) Cohorts provide space and place for HS grantees to come together to learn about what others are doing, document promising practices, and co-create tools and resources for the broader community. Cohorts provide small groups of HS staff (20-25) with opportunities to connect with other HS staff across the country, lift up and spread successful practices, and brainstorm solutions for common challenges.

The Healthy Start TA & Support Center is pleased to launch the first Healthy Start Evaluation Cohort! The goal of the Evaluation Cohort is to convene Healthy Start grantees – who are prepared to and interested in enhancing and strengthening their evaluation work – to create shared best practices for designing and executing data collection and evaluation processes. Healthy Start staff collecting and/or evaluating data are encouraged to apply. The Evaluation Cohort will meet on the third Wednesday of each month from 2-3:30p.m. EST from March through June 2021 (March 17, April 21, May 19, and June 16). 


This application will take 20-30 minutes to complete. Please note:
  • Please do not fill out this application on someone else's behalf (including for staff who you plan to onboard soon).
  • Please only complete this application if you are prepared to commit to participating in the Healthy Start Evaluation Cohort from March through June 2021. 
  • You must receive approval from your Healthy Start Project Director and your direct supervisor (if different from your PD) before submitting this application.
