ESO Clinic: Patient Feedback

Thank you for taking the time to complete our ESO Clinic Patient survey. Your feedback helps improve the clinical experience of patients and the teaching/learning experience of our students. There are key areas we would value your views on; space is provided at the end of the survey for any additional comments you wish to make. 

Please note, this survey should be used for general feedback only. If you have a complaint or a concern about the service you have received from a student practitioner, tutor or any of the staff working at the ESO Clinic, please refer to the Patient Complaints Procedure displayed at reception.
