Mamaroneck Veterinary Hospital Grooming Consent Form

Please review the information and answer the following questions to let our groomers know what services you wish for your pet to have during their spa day. 
A Bath Includes:
A Bath with our basic shampoo, a nail trim, an ear cleaning, and blow dry. 

A Grooming Includes: 
Bath service as above plus a hair cut. 

Please read the Hospital Policies below carefully: 
All pets must be current on vaccinations in order to be bathed or groomed.  If your pet is not vaccinated at Mamaroneck Veterinary Hospital, you must provide proof that your pet has received the requisite vaccinations from another veterinary hospital in order to maintain the highest health and safety standards for all pet-guests. 
These include:
  • For Dogs AND Cats: Up to date Rabies vaccination and negative Fecal Parasite Examination within the past 6 months.
  • For Dogs:  Kennel Cough vaccination within the past 6 months, and Distemper/Parvo vaccination up to date.

If your pet’s vaccinations are not current, s/he must be updated in order to be groomed. This policy is strictly enforced without exception, both for your pet’s health as well as that of other patients and pet-guests. Please ensure that we have your pet's updated vaccine records at the time of or before the grooming appointment.

*External Parasites:  Our hospital policy requires that all pets presented for grooming be on effective and current flea and tick preventatives.  eg: Oral products such as Simparica or Simparica Trio, or topicals such as Revolution Plus or Vectra, etc.

5. Does your pet have any medical conditions of which we should be aware?  *This question is required.
6. Occasionally, the groomer may notice a health problem with your pet (i.e, ear infection, skin condition, allergy, etc.). If so, may we have permission for one of our doctors to examine your pet?  *This question is required.
7. Please tell us how long or short you would like your pet's haircut to be (please note that a "puppy cut" is what your pet looked like as a puppy but shorter):  *This question is required.
8. Do you wish for your pet to receive any of the "Special Services" offered by our groomers? Please select which services you wish to have performed for your pet or select "None". 
If you wish to have your pet's anal glands expressed, please notify reception at time of drop off for grooming. This service is performed by a licensed veterinary technician or a veterinarian, and the cost is $47.00. All prices subject to change.  *This question is required.

9. What accessories, if any, would you like for your pet to get? Please select None if you do not want any accessories.  *This question is required.
10. Would you like your pet to be sprayed with cologne after their grooming?  *This question is required.
12. Grooming Consent and Acceptance of Hospital Policies: 
I certify that I am the owner of this pet and have the authority to execute this consent. Additionally, I have read the Hospital Policies and I accept responsibility for the charges for services rendered and agree to pay them in full before my pet is discharged.
If your pet’s vaccinations are not current, s/he must be updated in order to be groomed. Failure to provide proof of required vaccines or negative fecal screen for parasites will be treated as though your pet is not current on their vaccinations and they will be updated. I understand there are additional charges for these services which I agree to pay.
Any pets found to have fleas or ticks will be treated and I understand there are additional charges for these services which I agree to pay.
