After completing this brief questionnaire, more detailed information will be given to you before you decide whether or not to take part in the research, and you will have the opportunity to speak with and ask questions of a study coordinator. You can stop this interview at any time.
Please note that by completing this questionnaire, you consent to us collecting your answers to the questions, including your contact information.  All information you provide in this questionnaire will be held in strict confidence, and shared only with the research center conducting the study. Vital Research will delete your information from our records upon your request. While completing the questionnaire you will have the option to join our mailing list to be contacted via email about future clinical trials for which you may be eligible. You will not be contacted about future trials if you choose not to sign up to our mailing list.

Please note:
Due to the current situation with the COVID-19 pandemic, please note the following:
  • This is a virtual study, which means you will participate from the comfort of your own home.
  • There will be no in-person clinic appointments. The only in-person contact will be two visits to your home by a phlebotomist to draw blood samples. The rest of the communication will be via phone, email, or text. The study will collect 25ml of blood, and participants have the option to also provide an additional 50ml if they so choose.

Disclaimer: Emergency warning signs for COVID-19. If someone is showing any of the following signs to a significant degree, it’s a potential medical emergency, and therefore they should opt not to participate in this study and seek emergency medical care immediately:
  • Trouble breathing
  • Persistent pain or pressure in the chest
  • New confusion
  • Inability to wake or stay awake
  • Bluish lips or face
