USGA Performance Form

The USGA’s performance policy is designed to ensure that only those players truly capable of competing at a national championship level are participating in qualifying. The goal of USGA qualifying is to provide a pathway for players to earn a spot and compete for a national championship. Players who are not yet capable of competing at a national championship level are encouraged to play in their local, state and regional events prior to competing in a USGA qualifier.

Via this form, players may submit recent tournament scores that illustrate their ability to compete at the national championship level. Depending on which tier they fall in, these scores will be evaluated as follows:

Performance Violation 1
  • Minimum of four (4) total competitive scores with:
    • Minimum of one (1) multi-round tournament result that meets the following requirements.
      • An average score of 77 or lower.
      • Average score within 5 strokes of the Course Rating.
      • Qualifying and championship scores can be combined to satisfy multi-round events if they meet all other criteria
    • Single round tournament scores may be submitted to satisfy part of the requirements.
      • Any single-round score submitted must have a score of 75 or lower or be within 3 strokes of the Course Rating.
      • The format of the event must only contain 18 holes of stroke play to be used as a single-round score.
  • Club and team events are not acceptable.
  • One (1) result must be a tournament conducted by the USGA (Qualifying or Championship), Allied Golf Association, or PGA Section.
  • All submitted results must have online results that can be viewed by USGA staff. Scores acceptable for clearing a violation may be up to the discretion of the USGA Performance Committee.
  • Results must have occurred within the current year or last two calendar years.
The following combinations of single and multi-round scores can be submitted to clear a Performance Violation 1:
3 single-round scores
AND
1 multi-round score
2 single-round scores
AND
2 multi-round scores
1 single-round score
AND
3 multi-round scores
4 multi-round scores
Performance Violation 2
  • Minimum of six (6) total competitive scores with:
    • Minimum of three (3) multi-round tournament results that meet the following requirements.
      • An average score of 77 or lower.
      • Average score within 5 strokes of the Course Rating.
      • Qualifying and championship scores can be combined to satisfy multi-round events if they meet all other criteria
    • Single round tournament scores may be submitted to satisfy part of the requirements.
      • Any single-round score submitted must have a score of 75 or lower or be within 3 strokes of the Course Rating.
      • The format of the event must only contain 18 holes of stroke play to be used as a single-round score.

  • Two (2) results must be a tournament conducted by the USGA (Qualifying or Championship), Allied Golf Association, or PGA Section.

Within this form, you will be asked to provide:

  1. Title of Competition  
  2. Date of Competition 
  3. Score(s)
  4. A link to the tournament's web results
