USGA Performance Form

The USGA’s performance policy is designed to ensure that only those players truly capable of competing at a national championship level are participating in qualifying. The goal of USGA qualifying is to provide a pathway for players to earn a spot and compete for a national championship. Players who are not yet capable of competing at a national championship level are encouraged to play in their local, state and regional events prior to competing in a USGA qualifier.

Via this form, players may submit recent tournament scores that illustrate their ability to compete at the national championship level. Depending on which tier they fall in, these scores will be evaluated as follows:

  • Performance Violation 1
    • Minimum of three (3) multi-round individual tournaments with either:
      • An average score of 77 or lower.
      • Average score within 5 strokes of the course rating.
      • Average score within 5 strokes of their listed handicap.
    • Club and team events are not acceptable.
    • One (1) result must be a tournament conducted by an Allied Golf Association or PGA Section.
    • All submitted results must have online results that can be viewed by USGA staff.
    • Results must have occurred within the current year or last two calendar years
       
    Performance Violation 2
    • Minimum of five (5) multi-round individual tournaments with either:
      • An average score of 77 or lower.
      • Average score within 5 strokes of the course rating.
      • Average score within 5 strokes of their listed handicap.
    • Club and team events are not acceptable.
    • Two (2) results must be a tournament conducted by an Allied Golf Association or PGA Section.
    • All submitted results must have online results that can be viewed by USGA staff.
    • Results must have occurred within the current year or last two calendar years

Within this form, you will be asked to provide:

  1. Title of Competition  
  2. Date of Competition 
  3. Score(s)
  4. A link to the tournament's web results
Upon submission, the USGA Performance Committee will evaluate and determine eligibility within 7-10 days. Please contact performancecommittee@usga.org with any questions.
