The Student Health Survey (SHS) survey is a comprehensive, school-based, anonymous, and voluntary survey about health behaviors. It is a key part of statewide efforts to help local schools and communities ensure that all Oregon youth are healthy and successful learners.
Please help us make students healthier and safer by taking this survey. Your answers will help us understand what we can do to help you do better.
This is NOT a test. There are no right or wrong answers.
You don’t have to take the survey if you don’t want to. The first question of the survey will ask you whether or not you will participate.
The survey will take about 30 minutes to complete. Once you begin taking the survey, you will need to complete it (or complete as much as you can) before closing it. The survey does not allow you to save your work and come back later.
- Please be honest.
- You don’t have to answer any question you don’t want to.
- If you are not comfortable answering a question, you can leave it blank.
- If an answer doesn’t fit exactly, choose the one that’s closest.
- If you don’t know what a question means, it’s okay to skip it.
- Just because a question is asked doesn’t mean it’s assumed that you’re doing it or that it’s okay to do it.
