SFMTA Retiree/Dependent Pass

Please complete the following form for your annual Dependent/Retiree Transit pass.

For the first time, your annual pass will be available on the MuniMobile app. Once your application is reviewed and approved, you will receive a url link via to the email address provided below that will provide you with your annual Muni Pass.

If you do not have access to a smart phone you can opt to receive an annual sticker. Stickers will be sent via mail only to eliminate in-person transactions due to public health concerns.
