You should set aside 45 - 90 minutes to complete it. If necessary, you can use the save button at the top right side of this page (provide your email address) to save your answers and complete this questionnaire at a later time.

The information you provide is for internal purposes only to prepare your bankruptcy paperwork. We will review the final version with you for approval before we file it with the court. 

Disclosure is the key to a successful bankruptcy proceeding. You MUST disclose all your assets and creditors/obligations and have no discretion as to who/what information to include:
  • Minor omissions, such as failure to disclose $100.00 in your checking account, will likely be treated as a harmless error.
  • Failure to disclose $50K worth of gold in a safe deposit box, for example, can result in denial of Discharge or even criminal prosecution.
  • The point is, make an honest and reasonable effort to disclose information, and you should be fine.
The term Debtor refers to the person filing for bankruptcy and completing this form.   ​​  

Many of the questions require an answer:
  • Please type N/A if the question is not relevant to your situation.
  • Please type TBD (To Be Determined) if you don't understand the question or not sure how to answer it. 

Providing ESTIMATED values is sufficient in most instances. For example, you may list your car's approximate mileage or the approximate car payment amount if the exact data is not readily and easily available. Similarly, you may provide estimated values to your assets and liabilities.
 
Information for Married clients who are not separated 
 
Individual filing (spouse not filing) - the "Spouse" information should relate to the non-filing spouse. You are required to disclose your spouse's information to the extent that's relevant to your filing. That's mainly your spouse's income, which is part of your household income. 
 
Joint filing with your spouse - please designate one of you to be the"Debtor" and other "Spouse" and keep it consistent when answering questions.
