Compliance with the Communicable Disease Surveillance Protocols for Ontario Hospitals

In order to comply with the Communicable Disease Surveillance Protocols for Ontario Hospitals, the following Immunization records must be provided to SHN prior to your placement start-date.

If you are unable to provide the documentation of immunity, vaccinations and TB testing for the below-listed items, you, the student, will need to obtain your records or complete this testing at your family physician.

SHN Workplace Health and Safety (WHS) cannot provide this testing for students.
