Compliance with the Communicable Disease Surveillance Protocols for Ontario Hospitals

In order to comply with the Communicable Disease Surveillance Protocols for Ontario Hospitals, the following Immunization records must be provided to SHN prior to your placement start-date.

If you are unable to provide the documentation of immunity, vaccinations and TB testing for the below-listed items, you, the student, will need to go back to your family physician to obtain these records, or have completed at family physician.

SHN Workplace Health and Safety (WHS) is not covered to provide this to students.
