Youth Peer Survey: The Impact of COVID-19

Thank you for your interest in taking this survey! This survey is for middle and high school age youth living in the state of North Carolina.

The purpose of this anonymous survey is to look at any changes in the health and well-being of middle and high school age youth during the COVID-19 pandemic.  This includes things like the impact of changes in: the school/educational environment and experience, health including mental health, and access to healthcare. The results of this survey will provide the NC Division of Public Health (DPH) Adolescent Health Program with important feedback for next steps in supporting NC youth.  

This survey is a project of the NC DPH Youth Health Advisors, a group of young people ages 14-19 committed to identifying and amplifying the health needs of their peers, with support from their adult partners at the NC Division of Public Health. This survey was designed in consultation with the Kentucky Prichard Committee Student Voice Team, who have provided permission to use parts of their state survey. The NC Division of Public Health Youth Health Advisors have provided the final questions for this survey.

Responses will be used to inform work to support the health and needs of youth in North Carolina. The data will be discussed with decision-makers at both the NC Department of Health and Human Services and the Department of Public Instruction, as well as youth and parents/caregivers, and other statewide stakeholders invested in the health of North Carolina's youth (such as health care providers, behavioral healthcare providers, and educators).

The survey should take about 10 minutes to complete. You may skip any questions you prefer not to answer and can stop completing the survey at any time.

The answers you provide on this survey may be public record under North Carolina law. We kindly ask that you do not include any identifying information like names, dates, or locations in any of your answers.

Participation in this survey is completely voluntary and will not impact any eligibility for programs or services associated with the NC Division of Public Health or any partners.
