Parent Aware Addendum for Districts, Charter Schools, and Head Start

• Adding a new site or adding a partnership site.
• Address change - moving a currently rated site to a new location. 
• A site has permanently closed or a partnership has ended, and the site should be removed from your rating.
• Updating site details different from an address change (name change, adding/removing a DHS license or certification number, adding/removing a Child Care Assistance Program (CCAP) number, or correction of current site details). 

Note: If you are adding a new classroom to a currently rated site, you do not need to submit an addendum. As long as the classroom meets the standards for your program type (school readiness, VPK/SRP, Head Start), an addendum is not needed for opening a new classroom at a currently rated site.

Addendum Process:

1. Your program will complete this online Addendum Form to request the change. 
2. Once you click submit, the Minnesota Department of Children Youth, and Families (DCYF) will automatically receive a copy of your response. 
3. DCYF will review the requested change and will let you know if there are any questions or if your change has been submitted to the Rating Team. Within the form, DCYF may complete some "DCYF Use Only" fields. If this occurs, you may receive an email with an updated copy of your responses. 
4. The Rating Team will update your site(s), and will mail a new/updated Parent Aware certificate if applicable.
