Note: By team, we mean a group of staff who work together, and meet regularly to discuss caseloads, job development, business engagement, best practices, outcomes, etc. If your organization includes multiple teams, please respond about one team only, the one that might join this project.
4. For how many years has this team been providing supported and customized employment services?
5. What proportion of job seekers on this team's caseload are people with intellectual disabilities, autism, or other developmental disabilities?
6. What proportion of job seekers on this team's caseload are people from minority groups including Latinos, African Americans, Asians, Native Americans, Native Hawaiians, and Alaska Natives?
