CONGRATULATIONS! We are so honored to have you as a TelePal. This is the final step before connecting to your TelePal! We want to keep both you and our other TelePals safe, so we need you to read and acknowledge receipt and understanding of these items: TelePals handbook, Privacy Practices, and Confidentiality Agreement.

1. I acknowledge that I have received

a) TelePals Handbook: I agree to abide by the policies and procedures established in the training video and TelePals Handbook provided by Seniors in Service.
b). Notice of Privacy Practices: I understand that if Seniors in Service plans to use my personal information in a manner that is different than described by the notice, Seniors in Service must first get my permission.
c) Confidentiality Agreement: I agree to treat any information on clients, volunteers, and/or any other person/entity served by Seniors in Service as confidential. I will not disclose such information to anyone other than authorized persons. I understand that unauthorized disclosure of such information may forfeit my privilege of being a TelePal.
