Part 2

THIS FORM IS TO BE COMPLETED IN ENGLISH BY THE PARENTS/GUARDIANS.

I have read the Teen Family Handbook and agree to the expectations of the Commitment of Participation, including the following:
  • I give CFP permission to contact my teenager's school and/or teachers.
  • We the Parents/Guardians and the tenager undersand the purpose of the Program and agree to the teenager's spending four weeks in the United States with a partner of a different community under the cae of an American host family.
  • I agree that our son/daugther will treat the Programme and associated people with respect.
  • We understand that receiveing the CFP Program graduation certificate is a decision made by the CFP Coordinators based on the quality and behavior of our teens' participation.
  • We understand that our teenage will be matched with another young person of the same gender, but of a different community.  The purpose of this Program is for them to get to know and undersand each other within a neutral environment.  They will share a befroom while in the host family home and will hopefully build a lasting friendship.
  • We understand that early and ongoing contact between families of the paired teens is important for the Program's success and that parents/guardians should encourge the two young people who have been matched, to have as much contact as possible (including a visit to each other's home) before their departure date.  We understand that parents/guardians as well as teens must be willing to develope meaningful relationships.
  • We understand that, due to the bi-communal nature of the CFP Program, English is the language spoken at all times during the teens' entire CFP participation in their home country and/or the U.S.
  • I will observe all laws, including those related to the drinking and smoking age, at all CFP events. (This applies to the teen only, not natural parents)
  • I will honor CFP's bi-communal mission by supporting my teenager's need to attend CFP meetings held in the other community.
  • I will attend the graduation celebration with my teenager and understand that failure to do so may jeopardize the awarding of her/his certificate.
