Continuing Education Document

Instructions:

This information packet must be completed IN FULL to receive continuing education/contact hours based on the IACET 2018 Standard. Each CE packet must include the following:

Continuing Education Overview Form
Biographical Data Forms for EACH speaker presenting.
Declaration of Vested Interest Form for EACH speaker presenting.
Needs Assessment Form for EACH packet submitted. New instances (dates) and locations of the program will require an entire new packet to be submitted.
Education Outline Table Overview
Objective Evaluation Items
Description of Program (for Marketing) and ADA compliance review
A/V and Room Set-up Request Form for each presentation.

1. NOTE: If program event is longer than 1-2 hours and includes an agenda provided to participants, please provide a copy of the agenda with this packet. Education outline time frames and agenda time frames must match. Please upload your agenda here:
