Award Description:
The Harry Johns Award for Volunteer Excellence is announced at the Community Leaders’ Summit. Each year, the Association honors one volunteer who has demonstrated exceptional contributions and distinguished service that have impacted the entire organization.
Please be prepared to answer the following questions about your nominee:
- How has this individual demonstrated a contribution of lasting and continuing service to the mission of the Alzheimer’s Association, including any community, chapter, regional, and nationwide work, if applicable?
- Which values (accountability, agility, community, inclusivity, & integrity) of the Alzheimer’s Association has this individual exemplified in their service?
- Letters of recommendation (optional)
