Please try and coordinate internally and submit one answer per institution.   Please answer these questions based on what is true for your institution on the date you complete the survey.  We understand circumstances can change quickly and data may be different prior to the Forum on 4/30.  You may choose to complete this survey anonymously.  All results will be de-identified and reported in aggregate.

At the end of this survey, you may also submit any questions you have for the panelists in advance as well as suggest polling questions to be used during the Forum.
